Please note there is a £5 fee for GBP Cards and €7.50 for EUR Cards deducted from the balance

For any physical replacement cards being sent to countries outside of UK, there is an additional £12 fee for GBP Cards and €14 fee for EUR Cards deducted from the balance

N.B Once you have received your new card please send an email to cardexpirycardportal.com containing:

1: A Copy of this form completed and Signed.

2. A Photo of both sides of your new physical replacement card or a screenshot of your new virtual replacement card.

**it is only at this point the funds will be loaded on to the replacement card**
